Cancellations
I require 24 hours advance notice for the cancellation of office visits. This notice provides our office the opportunity to fill your spot, which otherwise may remain empty. Office visits canceled with less than 24 hours notice will result in a cancellation fee of $50.00.
Missed Appointments
Please provide me the courtesy of canceling if you cannot attend your office visit. Timely cancellation allows another patient to be able to see the physician. Missed office visits will result in a fee of $50.00.
Payment
This is a cash practice. I do not participate in health insurance plans. Payment is expected at the time of the visit and may be made through the online payment platform in my Charm EHR. In the case of a minor, the parent or guardian scheduling the visit is responsible for payment.

How should I prepare for my first visit?
It is a good idea to organize and record your past medical history in a timeline, including past illnesses/diagnoses, hospitalizations, injuries, current medications, allergies to foods, medications and environment, family medical history from grandparents to children if applicable and lab and imaging reports from the past few years. Much of this will be entered in the intake form when you establish a patient portal on Charm.
What should I expect from my first session?
I will go over a thorough past medical history including family medical history, social habits, diet, labs/imaging reports, history of present illness, and medications both prescription and OTC. We will do whatever physical exam permissible via telemedicine and will order labs or imaging procedures where indicated. If indicated at that time, I will give you a plan of treatment that will be sent by way of your patient portal in my Charm electronic medical record program. Where indicated, further referrals will be made.
Does Dr. Murphy accept my health insurance?
I do not accept any health insurances including Medicare and Medicaid (CT Husky). My practice is cash only and I will be prepared to accept payment with a credit or debit card online or over the phone through a payment service.
Will I need a computer with camera to participate in Zoom Telemedicine visits?
I prefer to see patients via Zoom Telemedicine, so you will need a computer/camera and an email on record for me to send you a Zoom invitation. For those without a computer and or the internet, phone consultations will be available at the same fee schedule.
